Clients briefly record the situation, thoughts (including images or memories), emotions and body sensations, coping responses, and the short-term consequences (helpful or unhelpful). The optional reflection prompts support pattern-finding and planning—what tends to trigger rumination, which responses work best, and what to try differently next time. Use it as homework or as a session tool to build awareness and guide next-step interventions.
